Query Criteria for Creation Date Not Working as Expected

I have a query ( in 7.2.4) that is designed to pull up incidents that are coded in a certain way (there are 4 variables) AND fall between two dates.

The query pulls up calls that meeting the first set of critera, but does not exclude incidents that fall outside of the range indicated.

 

Attached is a pdf of the critera I select. I am basing this query on the incident table.  Any insite will be greatfully received.

The query uses the OR's as parenthesis.  So it is showing all of the incidents from the first 3 groups.  and only filter out the dates on that last group.  In 7.3.1 there are some more advanced grouping features.
